Administration of Medication and Forms
If your child requires medical forms, has anaphylactic allergies or has diabetes, please print and compete the necessary forms below. Please bring these forms on the first day of school, along with any medications that the school may be required to administer.
Medication Forms: Required for all medications, including over the counter medication. Both sections of Form A are required for all medications. Form D can be completed in certain circumstances for self-administration of medication, but please confirm this with the school. Forms B and C are not required and are for school use only.

NOTE: It is important to recognize that medical information where appropriate will be shared with staff, including bus drivers. Pictures of students with anaphylaxis will be posted for staff only to view in the office area.

Food not Permitted in School due to Anaphylactic Reactions
Please note that we have severe anaphylaxis allergies in our school to:
EGGS
PEANUTS
TREE NUTS
BANANAS
KIWI
As a result, we cannot permit students to bring cooked or raw eggs, any products containing any type of nuts, peanuts or WOW Butter at school.
Please note: If your child has a diagnosed allergy by a medical doctor and you have not already done so, please complete the necessary forms.
